Step 1: Please submit this estimate request form for the tattoo that the minor wants. MAKE SURE THAT YOU DISCLOSE THAT THE TATTOO IS FOR A MINOR.

We do not tattoo minors under 16 years of age.

We do not tattoo minors above the collar or below the elbow unless it is a coverup or repair on an existing tattoo.

We will get back to you with estimates and booking instructions from artists that are willing to tattoo a minor.

Step 2: Please select either the Minor Tattoo Release Form or the Minor Body Piercing Release Form. Print out the form and take page 1 to a notary. The parent must have their
signature notarized stating they have legal custody of the minor that is requesting.

Step 3: Once you have received an estimate and booking instructions and completed the minor release form you can BOOK AN APPOINTMENT.

Make sure to exactly follow the booking instructions you received from our artist/receptionist.

Step 4: Please arrive on time on the day of your appointment. The parent/guardian that completed the notarized form must be present in the shop, the parent must have their legal, valid, photo id. We accept driver's license, state id card, military id, or passport as a valid form of id. The parent must bring in the completed notarized minor release form and must approve all details of the tattoo including but not limited to size, placement, etc.

The minor must bring in their ID (such as a school ID, driver’s
license, state ID, etc), the minor must be at least 16 years old. We do not tattoo minors above the collar or below the elbow unless it is a coverup or repair on an existing tattoo.
